When calling to request services, you will first talk with the Intake Support Specialist, to whom you will give information such as your name, address, telephone number, social security number, etc. You will also provide information concerning finances, employment, and insurance so that the cost of treatment can be determined.

The Intake Support Specialist will then arrange your first appointment. This will include the completion of a medical history form and financial paperwork.  The intake appointment includes an hour (approximately) of time with a licensed professional to review your social history, current presenting problem, and discussion of treatment options.  After completing the assessment, appropriate treatment will be arranged with professionals experienced in the areas necessary for your treatment.

Those with private insurance such as Anthem, Aetna, Medical Mutual, or other employer-sponsored plans are asked to call ahead to schedule their initial intake appointment so that they are matched with an in-network provider.
